Cost of Prospectus for all reserved categories (SC/BC/PH/FF/ESM/KM/HGST/GIRLS)

Application form, OMR (Optical Magnetic Reader) Form, Merit calculation form, Admit Card (for DET & DET (L) and all the details of counseling process.
How to fill up OMR form?
Item No. 1 Course Applying For
For filling up this column, first write in the box your course name for which you are applying (in English) in capital letters. Then darken the corresponding circle below each letter with HB pencil.
Item No. 2 Name of Candidate
For filling up this column, first write in the boxes your full name (in English) in capital letters exactly as recorded in your Matric Certificate. Write one letter in one box. Leave one box blank between name and sir name. Then darken the corresponding circle below each letter with HB pencil. Do not overshoot the boxes. Do not use any prefix such as Shri, Kumar., etc in front of your name.
Item No. 3 Father’s Name
Write your father’s name (in English) in capital letters strictly according to Matric certificate. Write one letter in one box. Leave one box blank between name and sir name. Do not use any prefix such as Shri, Dr., etc in front of Father’s Name
Item No. 4 Date of Birth
Write your date of birth according to Matric Certificate in the boxes using numerals 01 to 31 for Date, Jan. to Dec. for month of birth and last two digits for year of birth. Then darken the appropriate circle for the day, month and year of your birth.
Item No. 5 Sex
If you are Male, write MALE in the box and darken corresponding circle.
If you are female, write FEMALE in the box and darken corresponding circle.
Item No. 6 Rural/Urban
If you have passed 10th from urban school. Write URBAN in the box and darken corresponding circle.
If you have passed 10th from Rural school, write RURAL in the box and darken corresponding circle.
Item No. 7 Education Qualification
Tick the qualifying exam as per eligibility mentioned for the respective courses and darken the appropriate circle
Item No. 8 Code of Examination Station of your choice
For filling up this item refer code list of Examination Stations given for DET - 10 & DET - 10(L) in the chapter no.6 of Diploma Prospectus-2010. Write code name of examination station of your choice in the boxes where you want to appear for Entrance Test & darken the appropriate station code circle. (Candidates are informed that station of examination chosen by them may be changed, in case strength on particular station increases the maximum limit or the station is cancelled.)
Item No. 9 Photograph
Paste firmly in the space provided your recent passport size photograph of 3.5 cm x 4.5 cm(preferably in black and white). Do not staple the photograph. Photograph should neither be signed by you nor it should be got attested. This photograph should be latest and should be same on OMR Form and Application Form.
Item No. 10 Complete mailing Address of the Candidate with contact no.
Write your complete mailing address with pin code including your name in English capital letters with black ball pen only. Also write your contact number namely, telephone or mobile. Do not write outside the box.
Item No. 11 Signature of candidate
Make your signature in black ball pen within the box.
Item No. 12 Mother’s Name
Write your Mother’s name (in English) in capital letters strictly according to Matric certificate. Write one letter in one box. Leave one box blank between name and sir name. Do not use any prefix such as Mrs., Smt., etc in front of Mother’s Name
Item no. 13 Bonafide Resident of Haryana (can be seen at Annexure – I )
If you are bonafide resident of Haryana write YES in box and darken appropriate circle.
If you are not bonafide resident of Haryana, write NO in the box and darken appropriate circle.
Item No. 14 Category of Candidate
Write the category to which you belong in the box and darken the appropriate circle (refer RESERVATION in Prospectus for category code & Abbreviations on backside of OMR form)
Item No.15 Sub Category (If candidate is Haryana Domicile) Not applicable for All India Open (AIO)
Write the Sub Category to which you belong in the box and darken the appropriate circle (refer RESERVATION in Prospectus for category code & Abbreviations on backside of OMR form)
Item No. 16 Types of ESM Category (If candidate is Haryana Domicile & Sub. Category is ESM)
Not applicable for All India Open (AIO) : Write the types of category of ESM to which you belong in the box and darken the appropriate circle (refer RESERVATION in Prospectus for category code & Abbreviations on backside of OMR form)

Write the mode by which you have paid the counseling fees and circle the appropriate circle.

Write the only one district to which you belong & darken only one appropriate circle

Write your ten digit mobile no (don’t prefix ‘0’) & darken the appropriate circle.

Write the family income from all sources & darken only one appropriate circle.

Item No. 24 Declaration
The candidate and his/her parent with date and place must sign declaration.
